Dear Valued Patients,
We hope this letter finds you well. We are writing to inform you about some significant changes happening at Wexford Allergy, Asthma & Immunology, LLC.
Dr. Kapadia and Dr. Scholnicoff would like to thank you for the trust you have given them over the years as your allergist. Caring for you has been an honor for us as well as our staff. We are writing to inform you that Wexford Allergy, Asthma & Immunology, LLC, will be closing, effective August 30th, 2024.
We are excited to announce that effective September 5, 2024, Dr. Kapadia will be joining Advanced Allergy & Asthma in her new office, located at 6400 Brooktree Road, Suite 230, Wexford, PA 15090. She is looking forward to providing you with the same great care in addition to providing you with a broader range of services in allergy and asthma care at her new practice, should you choose to continue care with her.
We understand that change can sometimes be challenging, but we are committed to ensuring a smooth transition for all of our patients. We value your trust and are dedicated to maintaining the high standard of care you have come to expect.
Thank you for your understanding as we embark on this exciting new chapter. A copy of this letter and a medical records release form is being mailed through the U.S. Postal Service to all of our current and past patients. A message from Dr. Scholnicoff:
Effective September 1, 2024, I will end my clinical practice so that I can devote myself more fully to the needs of my family. I am very grateful to all of you for entrusting me with your care all these years. I hope that I have helped each of you in some way, and I will miss seeing you. I have known Dr. Kapadia for over 20 years, and it gives me great peace of mind to know that she will be available to continue your care should you choose to transfer your care to her. To transfer care to Dr. Kapadia, please return a medical records release form requesting your records be transferred to her new practice. I wish all of you the very best. If you wish to select another provider, please remember your health records are confidential. A copy of your records can be released to you or your new provider with your written permission with our medical record release form. Prior to August 30th, 2024, please return the completed and signed form to 100 Bradford Road, Suite 410, Wexford, PA 15090. There will be no charge for record copies transferred to another physician. After that date, Dr. Kapadia will be the guardian of the records, and copies can then be obtained for the fees set by the Pennsylvania Department of Health for reproduction of medical records by calling 724-719-2441.
