All patients must review Preparing For Your Visit at least 1 week prior to every appointment.
Please arrive 15 minutes prior to your appointment time with completed paperwork and other required documentation, including a current insurance card. Incomplete documents at the time of your appointment may result in rescheduling of the appointment and a cancellation fee.
TO AVOID A CANCELLATION FEE, PLEASE PROVIDE AT LEAST 24 HOURS' NOTICE WHEN CANCELLING OR RESCHEDULING YOUR APPOINTMENT. YOU MAY CALL THE OFFICE, USE THE CONFIRMATION TEXT RECEIVED ONE DAY PRIOR TO THE APPOINTMENT OR USE THE CANCEL APPOINTMENT LINK IN THE UPPER RIGHT CORNER OF OUR WEBSITE.
IMPORTANT: Having an appointment does not automatically guarantee you will have allergy testing. At the time of your consultation, one of our physicians will determine whether testing is safe and appropriate for your particular situation.
